We consider both the practical and clinical aspects of our clients' care, along with the emotional and social support required for daily life. Our extensive experience in delivering complex home care equips us to support our clients and their families effectively.
Our Registered Nurses are highly trained and experienced professionals registered with the Nursing and Midwifery Council (NMC). They utilize their clinical decision-making skills to ensure that our clients' care plans evolve in response to their changing needs and requirements.

At Uplifted Lives, our highly trained Healthcare Assistants provide comprehensive care at home, which includes:
- Supporting various clinical needs.
- Assisting with all activities of daily living, including personal care.
- Encouraging independence in personal care tasks.
- Utilizing technology to enhance independence.
- Facilitating attendance at social and educational settings.
- Addressing and intervening in challenging behaviors.
- Exploring new hobbies and promoting recreational activities.
Our extensive experience in delivering complex community care primarily focuses on clients with severe learning disabilities, known as Profound and Multiple Learning Disabilities (PMLD). Individuals with PMLD typically have additional disabilities or clinical needs that significantly impact communication and independence.
The conditions our clients may have include congenital disabilities, birth injuries, PMLD, Down syndrome, Autism Spectrum Disorder, cerebral palsy, and epilepsy. Many require specialized care such as oxygen support, tube feeding, or suctioning. Some have conditions described as life-limiting, while others have fragile health prone to issues like chest infections and gastrointestinal conditions.
Many of our clients with PMLD do not use traditional forms of communication such as speech, symbols, or signs. They rely on facial expressions, vocalizations, body language, and behavior to convey their needs. Our staff are trained to understand and interpret these forms of communication, ensuring effective interaction and support.
We have extensive experience in developing customized care plans that not only address clinical needs but also promote independence, empowerment, and choice.
A learning disability is a reduced intellectual ability and difficulty with everyday activities – for example household tasks, socialising or managing money, which affects someone for their whole life.
People with a learning disability tend to take longer to learn and may need support to develop new skills, understand complicated information and interact with other people.
